Cyclosporiasis is a parasitic intestinal infection caused by Cyclospora cayetanensis, typically linked to contaminated fresh produce. PrimeCare Urgent Care can test for and treat cyclospora โ€” and we can evaluate and manage your symptoms clinically even when testing kits are in limited supply.

WHAT IS CYCLOSPORIASIS?

A Produce-Linked Parasite That Is Easily Missed and Often Prolonged

Cyclospora cayetanensis is a microscopic parasite that infects the small intestine. It is most commonly acquired through eating fresh herbs or produce โ€” especially raspberries, basil, snow peas, and lettuce โ€” contaminated with the parasite’s oocysts. Unlike many foodborne illnesses, cyclosporiasis does not spread directly from person to person. Without treatment, symptoms can last weeks to months and may relapse. The infection is underdiagnosed because standard stool tests do not detect it โ€” specific testing is required.

Common Symptoms
  • Watery diarrhea โ€” often explosive
  • Frequent loose stools
  • Loss of appetite and weight loss
  • Bloating, gas, and stomach cramps
  • Nausea, sometimes vomiting
  • Fatigue and body aches
  • Low-grade fever
  • Symptoms waxing and waning over weeks

Symptoms typically begin 1โ€“2 weeks after exposure. Without antibiotic treatment, illness can persist for weeks to months and frequently relapses. Immunocompromised patients are at risk for more severe and prolonged disease.

Common Sources of Exposure
  • Fresh raspberries and other berries
  • Fresh basil and cilantro
  • Snow peas and sugar snap peas
  • Lettuce and mixed salad greens
  • Imported fresh produce (Latin America)
  • Restaurant meals with fresh herbs
  • Contaminated water in endemic regions

Most US outbreaks are linked to imported fresh produce. Michigan residents who have recently eaten fresh herbs, salads, or berries โ€” especially at restaurants or from grocery store salad bars โ€” and develop prolonged diarrhea should be evaluated for cyclospora.

Why Cyclospora Is Frequently Missed
Standard stool ova and parasite (O&P) testing does not reliably detect Cyclospora. Specific stool PCR panels or modified acid-fast staining are required. Many patients go weeks without a diagnosis, cycling through symptomatic treatments that do not address the underlying infection. If you have had prolonged or relapsing diarrhea after eating fresh produce, ask about cyclospora testing specifically.

We Test and Treat Cyclospora

PrimeCare of Novi can order stool PCR GI pathogen panels that specifically detect Cyclospora cayetanensis โ€” the same panels that detect other common parasites and enteric pathogens simultaneously. Testing is done via stool sample sent to our reference laboratory, with results typically available within 1โ€“3 business days. Standard O&P testing is not sufficient โ€” we order the appropriate panel to make the diagnosis.

The treatment of choice for cyclosporiasis is Trimethoprim-Sulfamethoxazole (TMP-SMX, Bactrim). A 7โ€“10 day course typically resolves symptoms in healthy adults. Immunocompromised patients may require longer courses. There is currently no well-validated alternative antibiotic for patients allergic to sulfa drugs โ€” patients in this situation will be managed on a case-by-case basis. Hydration and supportive care are also an important part of treatment.

At times, cyclospora-specific testing components or GI PCR panels may be in limited supply due to national or regional laboratory shortages. If testing is unavailable or results will be significantly delayed, our providers can still perform a thorough clinical evaluation โ€” reviewing your symptoms, timeline, dietary exposure history, and physical exam โ€” and treat empirically based on your presentation. A shortage of testing kits does not mean you will go without treatment. If your symptoms and exposure history are consistent with cyclosporiasis, we can initiate a course of TMP-SMX while awaiting confirmation or in lieu of testing entirely.
