What is included in a sports physical?

Bring your required school or sports form
Bring the correct form
Please bring the physical form required by your school, camp, league, or athletic organization. Parent sections should be completed before arrival when possible.
Know your child’s history
Be ready to discuss medications, allergies, surgeries, asthma, chest pain, fainting, concussions, injuries, and family history of heart problems.
Some findings may need follow-up
If we find something that needs further evaluation, we may recommend follow-up with your primary care physician, cardiology, orthopedics, or another specialist before clearance.
